The High Needs Patients Working Group leads metro-wide efforts to better serve the needs of high utilizers of the health care system, such as residents who frequently use emergency departments and are re-admitted to hospitals.  This costly over-use of the health care system could be prevented by relatively inexpensive early interventions and by connecting these patients to a primary care medical home.

In June 2015, the Group convened a Caring for High Utilizers Summit, bringing together stakeholders from high utilizer programs throughout the Denver metro region and governmental organizations to share information, plan for how to better coordinate programs, and learn how to best serve these vulnerable populations. The Group is working towards sharing data between high utilizer programs in Denver, tracking specific high needs patients, and developing a more formal learning collaborative around high needs patients.
